From the desk
For twin moms in the US, the Spectra S2 is the pump we recommend most often through insurance. Hospital-grade-style suction in a consumer pump, dual-channel, decently quiet, and most major insurers cover it under the ACA. That last point is the key fact: you can usually have an S2 delivered to your door for $0 by ordering through Aeroflow, Edgepark, or a similar DME supplier.
The S2 is plug-in only. The S1, its sibling, has a rechargeable battery for $50 more. Most twin moms we know who pump throughout the day pick the S1 for the freedom. If you'll mostly pump at home or at a desk with a plug nearby, the S2 saves $50.
Pump-parts inventory matters. Buy two full sets of duck valves and membranes per month. Get a flange fitting consultation; the default 24mm is often wrong. The pump itself is reliable; the consumables are what wear out.
Why we say buy one
One pump (yours), with two complete parts sets in rotation. Twin moms pump more, but they don't need two pump motors.

The decision behind this pick
Do I need a double electric breast pump for twins?
Yes if you're breastfeeding. Single pumps double the time you spend pumping. Most US insurance covers a double pump.
Why
Twin moms produce more and pump more. A double pump is a baseline. Hospital-grade rentals make sense for the first 2 months while supply is being established.
When to buy
Before birth; check insurance coverage.
